● Immunization records
● Birth Certificate
● Proof of Residency: 2 items such as:
o Utility bills (electric, trash, water, cable)
o Lease agreement or Mortgage bill
o Voter ID
o Tax receipt
**All utility bills must be current (within 30 days), in your name, for the household address
